on Website The Health Partners Pharmacy and Therapeutics
(P&T) Committee has reviewed and approved the Formulary
for 2008.
Please call the Pharmacy Department at 215-991-4300 with
questions/feedback. If you have suggestions for additions
to the Formulary, please submit these requests in writing
to the Pharmacy Department. Requests for additions to the
formulary will be reviewed by the P&T Committee. Health
Partners will continue to update the formulary quarterly and
communicate the changes to you in a timely manner.
You can also print out a copy of the Pharmacy
Drug Exception Request form by clicking on Pharmacy Drug
Exception Request. Please use this form if you are requesting
prior authorization to prescribe a drug that is not on the
formulary for a specific member.
