We want to help you help your Health Partners and KidzPartners patients to quit smoking and smokeless tobacco. For your convenience, covered products and services are summarized on this page.
Counseling to Quit Smoking
- Phone-based counseling services are available to any member enrolled in our Healthier YOU disease management programs, including asthma, CHF, diabetes and Fit Kids (pediatric obesity). Call 866-500-4571 to refer a patient for services.
- Counseling is also available to any member in our Baby Partners perinatal program. Call 866-500-4571 to refer a patient for services.
Pharmacy Coverage
Our formularies cover a wide range of smoking cessation products. Please note that we do not cover brand name drugs that have generic equivalents, except with prior authorization.
