| Find a Doctor |
Our Health Partners online directory gives you an easy way to find a doctor or other provider who participates with our plan. You can look for a provider by name or by location.
Please keep in mind:
- If you want to see a specialist, please go to your PCP to get a referral first (except for routine dental, vision or OB/GYN care).
- If you are looking for mental health or substance abuse providers, please call your county’s HealthChoices behavioral health organization. Phone numbers are listed in your Health Partners Member Handbook.
- Click the "HELP" button in the upper right corner on each directory page for tips on how to search. On the results pages, placing your mouse cursor on certain terms will also provide more information.
- Click the "More Search Choices" button on the first search page to add extra features to your search, such as the provider’s gender, language(s) spoken, specialty type, and hospital affiliation, or to limit your search by distance.
- On the Results page, you can see board certification for physicians, and whether PCPs are accepting new patients. You can also create and print a personal "short list" of providers by clicking the Short List "add" button for each provider you want on the list.
- This directory is updated monthly. Information is confirmed when the provider applies to join the plan’s network, and then every three years when the plan reviews ("recredentials") the provider for continued participation. Updates are also made whenever changes are received from the provider. Listings are subject to change without notice. For the most current information about providers listed here, as well as new providers, please call Health Partners Member Relations at 1-800-553-0784 (TTY 1-877-454-8477).
