| Each Health Partners member is assigned to a specific primary care provider, rather than a site or location.
Members can see any participating provider in that
practice, however, claims MUST contain the one specific
assigned primary care provider’s ID number (your
site number plus your medical license number.)
To avoid claim denial, you must be sure to confirm Member
Eligibility before every member appointment. |
