| Requirements for completing specific fields have
changed in the CMS-1500 and UB-92 forms. These revisions took
effect October 30th, 2000 and forms must be filed with this
new information regardless of the date of service.

Encounter Data Reminder
Please keep in mind that Health Partners must satisfy the Department of Public Welfare (DPW) reporting requirement for encounter data. Accordingly, Health Partners providers must provide complete encounter data on a properly completed HCFA-1500 form for each encounter with a Health Partners member. |
