Healthier YOU Care Management programs are your link to taking positive control of chronic health conditions.
Programs include:
- Asthma (adults and children)
- Diabetes (adults and children)
- Healthy Heart (congestive heart failure)
Our Healthier YOU nurses and other staff will keep in touch by phone and mail to help you understand and monitor your health issues. Partnering with you and your doctor, we can help you avoid complications and improve your overall health.
