Health Partners has implemented the CAQH (Council for Affordable Quality Healthcare) system for credentialing and re-credentialing of our providers. Click here to register or log in to the CAQH Universal Provider Datasource.
Physicians can apply to join the Health Partners and/or KidzPartners network as a Primary Care or Specialist provider by submitting a signed copy of the Provider Data Collection Form linked below, along with a signed copy of the Health Partners provider agreement. If you are on a group contract with Health Partners, we will require an updated Exhibit C with the new provider added.
We will continue to accept the Standard PA paper application below; however, we will also need to receive the Provider Data Collection Form. Completed documents can be sent via email to credentialing@healthpart.com, faxed to 215-967-9233, or mailed to:
