Please keep in mind:

• After you start, you will be able to narrow your results by choosing more search filters, such as gender, language(s) spoken, specialty type, and hospital affiliation, or to limit your search by distance.

• On the results page, click on “More about this provider” to see a summary of information about the provider/practice. You can also create a directory of providers that you can download or print, or have faxed to you.

• Click the "HELP" button in the upper right corner on each directory page for more tips on how to search. Click “Start New Search” at the top of the page to start over.

• Health Partners and KidzPartners members must have a referral to see a specialist (except for routine dental, vision or OB/GYN care). Only participating providers may provide specialist services without prior plan approval.

• KidzPartners behavioral health providers are included in this directory. For help to arrange these services, call CompCare at 1-877-710-8222. To arrange mental health or substance abuse care for Health Partners members, please contact their county’s HealthChoices behavioral health organization.

• Directory information is updated biweekly. Information is confirmed when the provider applies to join the plan’s network, and then every three years when the plan reviews ("recredentials") the provider for continued participation. Updates are also made whenever changes are received from the provider. Listings are subject to change without notice.
