Care Management Programs

Care management programs available to your patients who are members of Health Partners and KidzPartners include:

Asthma: For adults and children
Diabetes: For adults and children
Healthy Heart: Congestive heart failure program for adults
Fit Kids: Weight management program for children up to age 21
Complex case management services: For patients with multiple or complex conditions

Each program is based on collaboration with you, as our member’s healthcare provider. Our data indicates that individuals who are actively case managed have higher use of appropriate services and improved adherence to clinical practice guidelines for the specific disease condition

Your patients who are members of Health Partners and KidzPartners are automatically enrolled when they are identified by their utilization history. Of course, the programs are voluntary and members can call us to opt out.

Program features

Once a member is actively case managed, our staff sends a letter to you if you are the identified PCP, outlining all the services that will be provided as well as the member’s BMI, calculated by a case manager.

All care management programs offer benefit explanation, nutritional guidance by a licensed dietitian if needed, prescription coordination with community pharmacists and transportation assistance.

Our asthma, diabetes and healthy heart programs include mailings, appointment reminders and personal case management services to help members understand the best ways to self-manage their condition and periodically evaluate their health status.

For Fit Kids, our dietitian works with children and their parents or guardians to help promote healthy eating habits and encourage PCP follow up.

Adults and children who have complex needs receive assistance to overcome any barriers to care.
